Job Summary

As a Case Management nurse, you will work with a wide range of patients, including during hospitalization, within outpatient facilities, and in-home care settings, to develop a long-term plan of care. You will serve as the liaison in the coordination of primary care services and ongoing care needs. Your responsibilities will include managing the plan of care, booking appointments, following up with patients after appointments, providing resources, educating on medical conditions, and serving as the liaison between the patient and their insurance provider.

You must have a detailed understanding of nursing ethics and professional practice standards, relevant state regulations, and standard case management policies. You must also possess excellent communication, critical thinking, and organizational skills. Our ideal applicant is compassionate, steady under pressure, and capable of prioritizing when faced with the demand of high-acuity patients.

Qualifications

Education and Experience

Bachelor of Science (BS) degree from an accredited nursing program
A minimum of one year of case management experience preferred
Licensure and Certifications

Registered Nurse Licensure within the state of practice
Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification
