Job Overview:
A Case Manager administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and the plan of care under the direction and supervision of the Clinical Manager.

The Case Manager must have very strong interpersonal skills and the ability to build relationships with stakeholders, including staff and external partners, with emotional maturity.
A high degree of integrity is a must. Proven ability to maintain discretion, handle confidential information, and demonstrate the highest level of ethics and compliance. Exercises good judgment in a variety of situations, documents timely and with clarity and accuracy at all times to ensure regulatory compliance and integrity of work product.
Primary Responsibilities:

Provides services in accordance with the plan of care.
Makes the initial evaluation visit and regularly reevaluates the patient's clinical needs in accordance with specialty/scope of practice.
Initiates the plan of care and necessary revisions.
Provides services requiring substantial specialized clinical skills in accordance with specialty/scope of practice.
Initiates appropriate preventive and rehabilitative procedures.
Prepares clinical and progress notes for each patient visit and summaries of care conferences in a timely manner as per Agency policy.
Coordinates services, cooperating with other health, social, and community organizations providing or coordinating care.
Informs personnel of changes in the condition and needs of the patient.
Counsels the patient and family/significant others in meeting patient safety and care needs.
Participates in and presents in-service programs.
Understands and adheres to established Agency policies and procedures.
Processes orders and notifies the physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
Determines the amount and type of services needed by each individual patient based on clinical need.
Refers patients requiring specialized skills to Physical Therapists, Speech Language Pathologists, Occupational Therapists, and Medical Social Workers.
Supervises and teaches ancillary staff (PTA, COTA, LPN).
Conducts patient care conferences on patients assigned to their care.
Participates in peer review and performance improvement as assigned.
Participates in utilization review of medical records as assigned.
Gives total patient care as needed.
Takes on-call duty during nights, weekends, and holidays as assigned.
Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency-defined timeframes.
Appropriately utilizes ICD-10 codes.
Maintains knowledge and skills through active participation in training, in-services, and conferences.
Performs other duties and tasks as assigned by the supervisor to ensure optimal department functioning.

Requirements:

Bachelor's degree in Nursing with current NYS registration; or
RN with a bachelor's degree in health/human services; or
RN with two years' experience in home care
Acceptance of the philosophy and goals of this Agency
Ability to exercise initiative and independent judgment
