The Nurse (RN) is part of the Family Practice team that provides person-centered services addressing a breadth of beneficiary's needs, helping him or her achieve their personal goals.

Essential Duties and Responsibilities:

Manages the medication system in conjunction with the practice providers, administers and documents medication treatment;
Screens and monitors the beneficiary for medical problems and side effects (including nursing assessments, completion of AIMS);
Build rapport and engagement with beneficiaries
Engage in health promotion, prevention and education activities;
If the beneficiary is in agreement, develop strategies to maximize taking medications as prescribed (e.g., reviewing home environment to find clues to remind beneficiary to take medications; work with beneficiary and whole person care provider to scale back the number of times medications are taking each day);
Communicating and coordinating services with other medical providers;
Educating the team in monitoring symptoms and medication side-effects;
Direct and supervise LPN staff;
Accurately documents all billable encounters into Southeastern Integrated Care's EMR (electronic medical record) system within 24 hours. Any corrections will be entered within 24 hours of being notified.
Other duties as assigned.

Supervisory Responsibilities:

The Nurse (RN) position oversees Nurse (LPN) staff.

Qualifications:

Education/Experience

Minimum one year of experience working with adults with serious mental illness;
Associate's Degree of Nursing for (RN)

Required Skills/Abilities

Working knowledge of medications;
Ability to empathetically interact with beneficiaries and their families;
Strong clinical skills;
Operate independently or with minimal supervision.

Certificates, Licenses, Registrations

Valid NC Nursing License or Compact License that allows practice in NC;
Copy of a valid, unrestricted NC Driver's license, proof of insurance

The employee frequently is required to reach with hands and arms and stoop, kneel, crouch, or crawl. The employee is occasionally required to sit and climb or balance. The employee must be able to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Must be able to drive and operate a personal vehicle safely and adhere to all applicable state and traffic laws.
