Organization: VHCS Clinical - Nursing
Job Summary: This is a 32-Hour per week position. Collaborates in establishing the home health plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum. Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis. Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes. Employs strategies to promote health and a safe and healthy environment. The responsibilities listed are a general overview of the position and additional duties may be assigned.
Hours: This is a weekend position (Saturday and Sunday) plus 1-2 days per week, 8:00 a.m. - 4:30 p.m.
Department Summary: Vanderbilt Home Care provides licensed health care professionals in your home, the environment where you feel most comfortable and independent. Under the direction of your doctor, our nurses and therapists help you with daily tasks including bathing, dressing, eating, and managing medications. Our health care professionals also provide company and emotional support.
Signing Bonus Requirements: The position at 32-Hours per week, which is considered Full-Time, is eligible for the signing bonus. If the candidate selected works fewer hours, then the position is not eligible for the signing bonus. The signing bonus is not applicable to current Vanderbilt Health staff or former staff who have worked for Vanderbilt Health in the past twelve months. The signing bonus is only available for Full-Time positions. The signing bonus is paid out in two installments. You will be eligible for a taxable signing bonus of $10,000.00 paid to you with half ($5,000) at three months of employment and the other half ($5,000) paid to you at 1 year of service.

TECHNICAL CAPABILITIES Nursing Patient Education (Novice): Possesses sufficient fundamental proficiency in providing patient education in practical applications of moderate difficulty. Care Planning (Novice): Demonstrates ability to anticipate resource needs, identify distinct tasks, set priorities, schedule activities, meet deadlines, and organize work within areas of direct responsibility. Evidence-Based Practice (Novice): Possesses sufficient fundamental proficiency to successfully search for evidence to answer clinical questions. Generate New Ideas (Novice): Proactively identifies new ideas/opportunities from multiple sources or methods to improve processes beyond conventional approaches. Nursing Patient Assessment & Evaluation Home Care (Novice): Possesses ample fundamental proficiency to successfully demonstrate the ability to assess and evaluate patients in practical applications of moderate difficulty in the home setting.
