Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No guaranteed hours Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase (HCHB) and OASIS within the last year Job Description: - Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process.- Responsible to the Clinical Manager.- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.- Perform home visits and attend periodic office meetings.- Must have reliable transportation, a current driver's license, and agency-required liability insurance.- Manage an average of 5-8 patient visits per day.- Conduct various types of cases such as Start of Care, Wound Care, and Follow-up Visits.- Cover a territory with a mileage radius and be willing to travel up to 75 miles from the home clinic.- Document all patient encounters in a timely manner, within 24 hours of the appointment.- Accept all scheduled patients and ensure all documentation is completed to meet productivity expectations.- Participate in on-call rotations as required.- Local candidates accepted at the same rate.- Must be open to floating to nearby clinics if necessary to meet work requirements.
