Position Summary:
Responsible for coordinating the individualized care of patients throughout the continuum of care services, functioning as patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services.

Navigates patients through the continuum to eliminate barriers to health care and enhancing patient and physician satisfaction.
Working closely with our quality coordinator to ensure national registry data is collected pre and post procedure.
Working with the structural heart population which includes both complex adult inpatients and outpatients who are undergoing transcatheter valve replacements, Watchman procedures, and other catheter based procedures.
Function as collaborating member of the health care team, but also must be able to act independently to assess patient and physician needs to ensure appropriate hand-offs so that patients receive services promptly and efficiently.
Applies the nursing process as appropriate within the context of the organization's framework for professional practice and in accordance with guidelines established by the team.
Participate as needed in community education outreach programs/workshops/seminars/support services.






Qualifications Qualifications:



Bachelor of Science in Nursing - CT Nursing License
3-5 years' experience as a Registered Nurse preferred
Two years of patient/staff education experience preferred
Cardiology experience strongly preferred
