Devereux TCV Community Services is Immediately Hiring for a Registered Nurse for Outpatient Services Salary Range: $58,091/year up to $71,838/year (depending upon education and related experience).
Position Type: Full-time, exempt.
Schedule: Monday through Friday, 9a-5p.
Work duties and hours are based on the needs of the program and business operations.
$7,000 Sign-on Bonus Prospective employee hired for this position will receive a $7,000 sign on bonus.
The employee will receive the first third of the bonus after the successful completion of three (3) months of employment, the second third of the bonus will be awarded after the successful completion of six (6) months of employment, and the final third will be awarded after achieving a "satisfactory" rating or higher on the annual performance evaluation.
This position plays a critical role in fulfilling the mission of our Outpatient department and Devereux/TCV Community Services by providing nursing services to individuals served and nursing support to medical prescribers in an outpatient setting.
The chosen candidate will work as an essential member of the Outpatient treatment team using medically related and behavioral health skills to assist with the removal of related barriers to progress and promote mental health and physical well-being throughout treatment and crisis intervention.
In this position, you will be a key member of a multi-disciplinary team working with internal and external stakeholders of the organization in ensuring the delivery of quality services as per nursing license guidelines.
What to expect: Conduct mental health and physical health assessments as needed according to ANA standards of Nursing Practice and provide appropriate follow up.
Provide consultation/education to individuals in service, family members and other support staff as needed via phone or in person.
Develop and maintain a Treatment Plan which reflects the individuals needs and goals.
Review and update Treatment Plan with multidisciplinary treatment as per department regulations.
Prepare and administer medications as ordered by physician in accordance with TCV policy and nursing license regulations.
Assist in safeguarding medications stored on organization premises in accordance with policy.
Assist medical prescribers in assessing need for medication refill and any potential adverse reactions.
Assist medical prescribers with ordering any diagnostic testing.
Monitor laboratory results.
Inform prescriber of any abnormal lab results or health related issues.
Recommend medical follow-up when appropriate.
Assist individuals with arrangements for medical follow-up when necessary.
Complete any needed prior authorizations in order for individuals to obtain prescription medications and or treatments.
Act as the consultant between behavioral health and physical health providers in order to improve coordination of care.
Assess crisis and respond to TCV crisis codes.
Assist is coordinating appropriate plan of action.
Adhere to all privacy regulations and laws.
Act in compliance with nursing licensure regulations and requirements.
Display a working knowledge of TCV and community resources.
Refer individuals needing other services.
Contact collateral resources such as physicians, other organizations, and supportive services.
Collaborate with pharmacies, physicians, laboratories, service providers, schools, etc.
Maintain clinical records.
Document contacts with individuals in service and collaterals.
Update record/forms as needed and reviews ECR/charts for compliance with County and State Regulations.
Complete all assigned trainings and stay up to date with current safety standards.
Employs universal blood and body fluid precautions as recommended by center for disease control when administering medications or providing direct care.
Maintain clinical competence by utilizing supervision, current literature, consultation, and staff development opportunities.
Qualifications: Graduation from an approved nursing program.
Current PA RN license.
One (1) year of nursing experience in an outpatient behavioral health setting required.
Valid PA Driver's License required.
