As a Registered Nurse you will:Assess and evaluates patient needs/problems, identifies mutually agreed upon goals for care plan with patientsReport patient status, updates and discharge planning process to clinical leadership, attending physician and hospice physicianAttend staff meetings and educational in-services as neededParticipate in Quality Assessment and Performance Improvement (QAPI) program through submission of data collectionBe responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care QualificationsCurrent unencumbered registered nurse in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), andMust maintain a valid driver's license and good driving recordAbility to work in a field setting and exhibited ability to make sound nursing judgmentAbility to assess patient needs and formulate individualized patient care plans to meet those needsExperience with Microsoft Office Pay Transparency Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
