Assess & evaluate the physical and psychological wellbeing of the cardiac rehab patient, monitor the exercise plan and help the pt. to achieve progress towards mutual goals of risk factor identification and modification to long term plan and assessment during rehabilitation. Participate in data collection for pt. To coordinate, implement, and plan nursing care in a timely and accurate manner.
PERFORMANCE STANDARDS
Formal Education / Training:
# Earned a nursing diploma or degree from a college or university.
# Patient Care Delivery: Prefer experience in out-patient cardiac rehab with the ability to educate patients and families on risk factor modification for heart disease.
Equipment and Skills Training:
The ability to work well under pressure, to function dependently and independently and to be flexible in applying basic nursing knowledge to a variety of setting. Equipment: Cardiac Monitor, Treadmills, Exercise Bicycles, Rolling Machines, Stairmaster, Exercise Weight Machines/weights.
A cardiac rehab unit with patient population ranging from less than one year of age
Requires the ability to communicate in English orally and in writing.
# Requires the ability to lift and turn patients with assistance up to 250 lbs.
PERFORMANCE STANDARDS
# Develops plan of care based on patients' history and medical diagnoses.
# Conducts and implements exercise prescriptions according to AACVPR and Williamson Health guidelines.
# Provides regular patient reports to referring physicians and Medical Director.
# Monitors and maintains department equipment including but not limited to exercise equipment, telemetry monitors, oxygen saturation monitors, and LSI documentation system.
