Massachusetts General Hospital Eating Disorders Collaborative Care Clinic (EDC3) provides expert, specialized medical and psychiatric care for adolescents and adults with eating disorders. The Ambulatory Nurse is a Registered Nurse who is responsible for assuring competent, compassionated, individualized, nursing care for patients and loved ones. The nurse will collaborate with all members of the multidisciplinary care team, providing coordinated patient care. This 0.5 FTE (20hr/week) position will be in-person. The EDC3 Nurse will provide clinical triage of new patient referrals, and review and follow-up of clinical messages and phone calls from patients to the EDC3 interdisciplinary team. The RN may also conduct nursing visits with patients, obtaining weights and orthostatic vitals when needed. In addition, the EDC3 Nurse will develop a database of resources for patients requiring outside care and assist patients seeking out these additional supports (including therapists, support groups, higher levels of care). The RN will provide direct care collaboration with eating disorder programs and community providers when applicable. This position will support the mission, vision and values of the Eating Disorders Collaborative Care Clinic. The RN will serve as a member of an inter-professional team, and promote partnership between patient, loved ones and team. Primary responsibilities Provides proficient, compassionate patient-and-family-centered evidence –based nursing care applying the nursing process and interdisciplinary teamwork, and collaboration. Applying their assessment, critical reasoning and astute clinical judgment, the nurse expedites appropriate care and treatment of the patient. Documents, in a timely manner, nursing assessment, interventions, plan and outcome in the patient record. Crafts a therapeutic relationship with patient and family. Applying telecommunications technologies, the nurse assesses, triages, provides nursing consultation, and performs follow-up and surveillance of patient status and outcomes. Conducts in-person nursing visits with patients, obtaining weight and orthostatic vitals when needed. Provides health education about eating disorders to facilitate patient treatment plan. Respects and supports patient and family's cultural beliefs and values, individual needs, health goals and treatment preferences. Collaborates with all members of the health care team to ensure coordination of care. Reviews and triages new referrals to the Eating Disorders Collaborative Care Clinic. Communicates any urgent recommendations with referring providers and PCPs. Reviews treatment recommendations with patients and encourages appropriate follow-up care. Tracks and follows-up with care team and patient on critical test results. Assists with referrals to higher levels of care for further eating disorder treatment. In collaboration with the health care team, evaluates efficiency of care and interventions for patients. Communicates with members of the health care team-in-patient and/or in the community- to ensure continuity in care transitions. Actively engaged in crafting a healthy, respectful work environment. Provides leadership in the practice to ensure safe, efficient and cost effective care. Identifies opportunities, and solutions, to ensure a safe, efficient and cost effective practice environment. Demonstrates compliance with regulatory requirements Participates in quality and safety initiatives Generates innovative ideas to improve care delivery and workflow in the practice environment. Precepts and mentors new members of the team. Participates in departmental and organizational committees and initiatives.
