To develop a plan of care to meet the patient's needs based on the comprehensive assessment. To report findings to referring physician in order to coordinate care. To provide care as needed and/or supervise care provided by the HHA. To delegate duties to the HHA when appropriate. Able to communicate with staff and physicians regarding patient progress. Able to complete required documentation within company policy. Able to work independently and make decisions related to patient care.
