You should be able to address all the caregivers' concerns and make appropriate adjustments to the patients' in-home management plan. The ideal candidate will provide quality, skilled and/or palliative patient care to our home-bound clients, conduct home visits as assigned, assessing the patient's condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered, while adhering to State Nurse Practice Act, agency policy, and accepted professional standards of practice, while providing nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.*Job Responsibilities:*· Traveling to patients' homes and managing their care plans according to physicians' instructions.· Administering medication and insulin, and completing blood pressure, glucose, urine, and stool tests.· Inspecting wounds, changing dressings, and handling personal grooming and hygiene.· Testing for muscle weakness, bedsores, and any signs of infection.· Listening to the concerns of family members and answering their questions.· Educating caregivers and family on the aftercare or ongoing care of the patient.· Providing suggestions for improved healthcare to physicians and family members of the patient.· Monitoring patient recovery and compiling reports for the physician.· Keeping abreast of developments in healthcare and attending workshops and lectures as required.· Collaborating with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients.*Experience:** Current license as a Registered Nurse in NM.* Graduate of an accredited school of professional nursing. BSN, PHN preferred.* Minimum of one (1) year experience in an acute care setting or equivalent experience.* Current driver's license, auto insurance, and reliable transportation.* CPR certification.* Strong communication and interpersonal skills.* Homecare/Hospice experience preferred.* Mountain Home Health Care offers a competitive benefit package including health, dental, vision, life insurance, paid time off accrual, paid holidays, short term disability as well as many more*Home health Nurse Requirements:*· Active RN or BSN License· Relevant license and certification.· Excellent observational and problem-solving skills.· Nursing skills as defined as generally accepted standards of practice· The ability to follow instructions but also to act independently if necessary.· Excellent report writing skills.· Active listening skills and empathy.· Physical and mental strength.· A valid driver's license and reliable transportation, insurance required.*Education:*· Completion of an accredited Registered Nursing Program.· Preferred, BSN.· Job Type: Full-time· Salary:Staring $36.00 per hour#INDZYJob Types: Full-time, Part-time, PRN, Per diemPay: From $40.00 per hourBenefits:* 401(k)* 401(k) matching* Dental insurance* Employee assistance program* Employee discount* Flexible schedule* Health insurance* Life insurance* Paid time off* Vision insuranceMedical Specialty:* Home Health* Hospice & Palliative MedicineWork Location: Multiple locations
