Responsibilities
• Provide quality healthcare to patients in private homes• Participate in development and revisions of care plans• Patient and family education• Collaboration with patient's care teams• Participate in education programs• Mentoring and educating co-workers• Record and report changes in patients' condition• Use effective relationship management techniques• Communicate updates of medical condition to Physician and Clinical Director• Other assigned duties
Requirements
• Current RN licensure with the State Board of Nursing in the State of New York• 1-year experience as a Registered Nurse in the last 3 years• Current CPR certification• Negative TB/PPD or Chest X-Ray within the last 12 months• Successful completion of skills assessment• Qualify for and complete Nurse Residency Program (if no experience)
