Founded in 2008, Alianza Family Services is a licensed service provider under the New Mexico Developmental Disabilities (DD) Waiver Program.Alianza Family Services is seeking a self-driven, team-oriented, full-time RN to join our growing nursing team. The Agency Nurse evaluates, assesses, and assists with planning for the health, safety, and well-being of DD Waiver clients in home-based settings and the community. This position plays a vital role in ensuring compliance DD Waiver Service Standards while ensuring clients receive quality care and services that align with their individualized service plans.Key Responsibilities:* Conduct initial and ongoing assessments of individuals' health status and care needs.* Develop, train, and monitor individualized care plans for families and direct care staff.* Educate individuals, families, and direct care staff on health conditions, medication management, and preventative care.* Advocate for healthcare needs of individuals within the community and healthcare settings.* Regularly monitor individuals' health, and adjust care plans as needed.* Triage and respond to health-related concerns, and provide guidance to direct care staff on proper response protocols.* Work collaboratively with other members of the interdisciplinary team, including Guardians, Case Managers, Physical Therapists, Occupational Therapists, Speech Language Pathologists, Behavioral Therapists, and other Medical Professionals.* Ensure compliance with DD Waiver Service Standards.* Maintain accurate and timely documentation of all assessments, care plans, interventions, and communications in compliance with state and federal regulations.Qualifications:* Registered Nurse (RN) with a valid New Mexico license.* DD Waiver experience preferred, but willing to train the right nurse.* Bilingual preferred, but not required.* Strong assessment, clinical, and critical thinking skills.* Excellent written and verbal communication skills.* CPR certification.This position requires initial training for up to 3 months. The training schedule is 4 days/week in office, and 1 day working remotely. After training, the schedule will be 1 day/week in office, and 3 days working remotely, with Friday rotation in-office.On-Call: Agency Nurses are on-call for their assigned client caseloads after regular business hours and on weekends for emergencies only.
