*Full job description*Home Health and Hospice of Montana is a growing organization that provides support and quality health care to the homes of our patients all throughout Missoula County and the Bitterroot. We offer home health, hospice, palliative care, and our team is dedicated to having a positive impact throughout our patients their healthcare journey. Our goal is to earn the trust of patients while providing them with the best healthcare and improving their quality of life.*Responsibilities/Duties*· Some of the duties for the home health/hospice RN include but are not limited to:· Providing skilled nursing and coordinating care to patients and patients' families within their home whether that be a house, apartment, or facility. We cater to a range of different living situations and home lifestyles.· Monitor, assess, evaluate and plan interventions regarding the patients' physical and mental statuses.· Executes continuity of care through thorough communication, information exchange between clinicians and administrative staff. Floats between home health and hospice as needed.· Creates Aide Care Plans on both the home health and hospice side for the Home Health Aides. Oversees care provided and ensures compliance with care.· Being the main participant in the team scheduling and "on-call" rotation as necessary. RN will assess the patients' care needs via telephone and triaging as necessary.
