Mass General Brigham is seeking a Registered Nurse (RN) Med Surg for a nursing job in Somerville, Massachusetts. QUALIFICATIONS: - Education: Graduate of an approved school of nursing with current registration in Massachusetts. Bachelor of Science Degree in Nursing is required for newly licensed nurses. - Experience: - Minimum of 2 year(s) of recent (within the past year) clinical nursing experience working primarily with patients with cognitive impairments and dementia is required. - Minimum of 2 years of recent (within the past 6 months) clinical nursing experience in an Ambulatory Clinic/Practice setting is required. Experience only in an ambulatory procedural area or ED is not sufficient. - A strong understanding of and interest in working with patients with cognitive impairments and dementia, inclusive of the impact on patients and families as well as related treatments and interventions, is required. - Recent experience with telephone or video-based and/or in-person clinical assessment techniques is required. (This is the safe, effective, and appropriate disposition of health-related problems and includes: assessing the severity of symptoms, communicating urgent patient needs to the LIP, referral to the appropriate setting or level of care, documenting interventions, and an individualized plan of care using an EMR, education of patients and families on their disease process and interventions to promote health and well-being, preliminary interpretation of data, providing home health advice and follow-up, ability to make triage decisions quickly and calmly in emergencies, and assuring continuity of care.) - Clinical Research experience preferred. SKILLS/ABILITIES/COMPETENCIES REQUIRED: - Ability to multi-task, prioritize effectively, and remain organized and detail-oriented while completing tasks is required for success in this role. - Membership in professional organizations is recommended and nursing certification in Ambulatory Care or an Ambulatory Specialty area is preferred. - Able to work weekend rotation per the MNA contract. The Center for Brain/Mind Medicine at Brigham and Women's Hospital (BWH) specializes in navigating the complex cognitive, behavioral, and social challenges that accompany changes to the brain and mind due to neurologic injury or disease. Our mission is to help individuals with neurologic and neuropsychiatric conditions live full and meaningful lives. We are seeking to hire a nurse who has a strong interest in providing care navigation for longitudinal patients who have progressive neurodegenerative conditions with dynamic care needs throughout the evolution of their illness. PRINCIPAL DUTIES AND RESPONSIBILITIES: - Delivering patient-centered care in the Cognitive Neurology Clinic through direct communication with patients and caregivers. - Engaging family members and others interacting with patients to assess and navigate changes in medical conditions, symptom management, and medications. - Participating in obtaining pertinent clinical data from patients and caregivers prior to and during clinic visits. - Collaborating in providing patients and caregivers informational materials specific to their diagnosis, stage, and needs. - Ensuring smooth execution of plan elements including completion of labs and studies, obtaining prescribed medications and pertinent education, connection with referral resources, and scheduling of return visits. - Communicating with patients and caregivers between visits, both to follow up on pertinent elements of plan (e.g., response tomedication, compliance with behavioral and other non-pharmacological recommendations) and to address unexpected issues (with support from MD). - Communicating with other care team providers within and outside BWH/MGB, including but not limited to, geriatric psychiatry, neuropsychiatry, neuropsychology, social work, other neurological and medical sub-specialties. - Interfacing with the MGB Alzheimer Therapeutic Program (ATP), our program for delivering new disease modifying therapeutics to patients with Alzheimer disease, to ensure seamless care across clinics. - Interfacing with the BWH Center for Alzheimer Research and Treatment and the Massachusetts Alzheimer's Disease Research Center to promote engagement with research studies, where appropriate. - Working collaboratively to provide excellent care to patients and families in the safest environment. - Other duties as assigned by the supervising physician or leadership team.
