The objective is to assist in improving quality of care outcome measures by ensuring the integrity of the OASIS data bank. This utilizes McKesson Horizon, Crystal, SHP and/or PPS Plus (or other software as available) reports, alerts, and validations to ensure OASIS data is accurate, consistent and complete and also utilizes other Palmetto Health computerized systems such as EpiCenter and Centricity in obtaining and needed clinical information. Be the difference.Responsible for timely administrative review of clinical records providing OASIS and Plan of Care review for accuracy and congruency, and clinical documentation review to ensure patients admitted to the Agency meet admission criteria according to regulatory, payer and admission policies. Maximizes use of software applications to complete the review. Collaborates with team members and providers to ensure accurate clinical record documentation requirements and development of effective Plans of Care. Elevates issues appropriately.Prepares comprehensive reports of clinical record review analysis including OASIS documentation/practice patterns that may result in potential adverse patient outcomes, unfavorable percentage rates in the Quality of Patient Care process, outcome and utilization measures.Provides education to team members for clinical record documentation requirements.Performs other duties as assigned.Supervisory/Management ResponsibilitiesThis is a non-management job that will report to a supervisor, manager, director or executive.Minimum RequirementsEducation - Graduate of an accredited school of Nursing program approved by the South Carolina State Board of Nursing. Bachelor's degree in Nursing preferred. Experience - Two (2) years post-acute care experience.
