This RN works 40 hours weekly, 8 am to 5 pm, Monday through Friday. Delegates aspects of care to other nursing personnel based upon their licensure, preparation and job descriptions. Contributes to the meeting of the mission and goals of the facility and Essentia, and meets the requirements of the Joint Commission if applicable and/or other federal, state and local regulatory or accrediting agencies. Responsible for care coordination, patient education, and decision-making support for patients. Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Essentia and in the community. Participates in conferences and committees, assisting the entire care team in developing optimal care plans for each individual. Collaborates with other service providers/organizations and community supporters in providing patient education and outreachLicensure/Certification Qualifications:Current Registered Nurse license in the state of North Dakota upon hire.RN License in the state of Minnesota within 3 months of hire.Basic Cardiac Life Support (BCLS) certification within 1 month of hire.2 years of related experience required.
