Be the difference.Job SummaryProvides care and consultation for a specific patient population with alterations of wounds, the GI/GU systems, drains, pressure ulcers and incontinence. Serves as a resource for other health care professionals, patients, families and the community. Designs systems to facilitate the appropriate care of this patient population for the Greenville Health System.Accountabilities* Assesses the physical and educational needs of patients undergoing ostomy surgery or continent diversions, or patients with wounds, fistulas, incontinence, percutaneous tubes or pressure ulcers. Communicates patient needs to appropriate members of the health care team. Provides discharge teaching as requested. 20%* Identifies opportunities for system-wide improvement in skin, ostomy, wound or continence care; develops/revises Nursing Policies and Procedures, Standards of Care, and protocols based on current research. Evaluates the outcomes and effectiveness of system wide improvements. 15%* Develops plans for care management based on patient needs through consultation and collaboration with other members of the patient's health care team; coordinates care in an efficient, cost effective manner. 15%* Based on protocols, procedures and guidelines, implements and evaluates the plan of care for individual patients.15%* Identifies actual/potential issues of resource management/patient care, works with health care team to identify cost-effective alternatives; facilitates discharge. 15%* Educates patients, families and staff on evidence-based skin and wound care practices. Promotes health care, in area of expertise, in the community through education. 5%* Promotes role identification by serving as clinical preceptor to undergraduate nursing students and staff. 5%* Coordinates meeting agenda and issue follow up for the Prisma Health Skin Care Committee. As a member of the Committee, evaluates products used, including specialty beds and mattresses and presents findings and makes recommendations to Prisma Health Skin Care Committee and Products Committee. 5%* Coordinates quarterly Pressure Ulcer Prevalence Studies and facilitates performance improvement activities related to analysis of results and serves as a resource. 5%Supervisory/Management ResponsibilitiesThis is a non-management job that will report to a supervisor, manager, director or executive.Minimum EducationBachelor's Degree - NursingMinimum Experience3 years - RN experienceRequired Certifications/Registrations/LicensesRN - REGISTERED NURSE - NLRNCertified by WOCNCB as a CWOCN, CWON, CWCN or COCN (Lawson Code COTR)Current South Carolina R.N.
