Reporting to the Regional Director, Operations, the Resource Nurse (LPN) will support communities within the local market to offer nursing support as needed at communities where there is a staffing need.
This Resource Nurse (LPN) will assist the Wellness Director with planning, coordinating, and managing the home care service and act within the State Regulations and Sonida Policies and Procedures.
This LPN is responsible for the overall direction, coordination, and monitoring of care service delivery under the direction of the Wellness Director.
*Ideal candidate will be located in or near southern Ohio.Resource Nurse LPN Position Responsibilities:Works to assure facility is in compliance with federal state and local standards and regulations.
Is available for license surveys.Works to maintain privacy and confidentiality of client, client's records and client living environments.Is willing to carry a cell phone and be available for staffing and notifications.Administers medications to residents as prescribed by physicianAssures clients maintain residency requirements, monitors and recommend alternative placement if necessary.Assist care staff as needed.Monitors and maintains client charts and nursing notes.Communicates and documents client data to staff as needed.Communicates to physicians any changes in client's needs or conditions, as needed.Monitors and follows thru with ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications.Maintain client files and appropriate documents as required.Assures clients continue to live safely in their unit, monitors and recommends alternative care and services, if necessary.Develop and maintain a positive working relationship with staff of Capital Senior Living, including providing backup to other staff as needed.Maintains communication with clients, families, and other staff.Responds to client concerns or complaints.Keeps Wellness Director informed and involved and advised of needs and problems.Qualifications and Requirements:Must be licensed with the State of Ohio as an LPN, and must maintain current and unencumbered licensure.Must have a valid driver's license, if driving is required.Ideal candidate will be located in southern Ohio.Subject to completing TB test resultsAbility to comprehend and apply federal, state and local standards and regulations to maintain compliance.Provide support and address needs for programs and services.Participate and function effectively as a team member.This position requires travel at least 75% of the time.
