This is an excellent opportunity to join a nationally recognized team.Position Summary: This is a 7 day on/7 day off, On Call position. Monday-Monday. Weekday hours are 5pm-8am and Weekend hours you field calls during day and make visits from 5pm-8am. Hours are flexible and can be discussed further at interview process. The registered nurse plans, organizes and directs hospice services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.Why work for Affinity?Paid 4 PTO weeks in the first year.Paid MileageHealth and Dental InsuranceEmployee Assistance Program Internal Company AdvancementFree end of life trainingTuition reimbursement for qualified team members.401KResponsibilities:RN Essential FunctionsCase manages and provides clinical direction to the interdisciplinary team, physician, and family.Implement/develop/document the patient plan of careProvide care utilizing infection control measures that protect both the staff and the patient (OSHA).Assure continuity of quality patient care delivered with appropriate documentation.Monitor assigned cases to ensure compliance with requirements of third-party payer.Demonstrate commitment, professional growth, and competency.Promote agency philosophy and administrative policies.Perform on-call responsibilities and provide on-call service to patients/families as assigned.Supervises and provides direction to the home health aide and the LPN.Qualifications:RN Job RequirementsEducation: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.Licensure: Current State license as a Registered Nurse, current Driver's LicenseExperience: Two years of experience as a Registered Nurse in a clinical care setting, home health or hospice preferred.Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of CPR, Hepatitis consent/declination, and Covid Vaccination or eligibility for a religious/medical exemption.Transportation: Reliable transportation. Valid and current auto liability insurance.
