Job SummaryThe RN Adminission / Discharge Coordinator assesses, plans, implements and evaluates the nursing care of all admissions and discharges at the RNC.  Responsible for ensuring, continuity of care of the residents between shifts by providing direct care as well as guiding the care given by CNA's, LPN's and supportive staff members.  Participates in resident and family teaching/conferences.  Maintains a role as resident advocate with a focus on the facility's mission.Core Job ResponsibilitiesAssesses residents on admission, readmission as well as when condition changes. Implements plan of care for the resident based on assessments and goals as established by the interdisciplinary care team.  Evaluates plan of care for effectiveness and revises plan of care as indicated by the resident's response to treatment.Coordinates labs, appointments, and special instructions on all admissions and discharges.Identifies and interprets clinical changes as they relate to medical diagnosis. Questions or clarifies medical orders for resident's condition.Notes and carries out physician's orders in a timely manner.Notifies physician of changes in resident's condition and follows through until appropriate action is taken.Provides thorough reports to oncoming shift and participates in walking rounds.Assesses and reassesses pain.  Utilizes appropriate pain management techniques.  Educates the resident and family regarding pain management.Performs all duties as a staff nurse when needed.Submits written documentation in a timely, clear, concise and pertinent manner.Keeps leadership informed of any issues, problems or concerns timely.Follows through on information given by shift report, resident or family regarding resident concerns.  Coordinates with other members of the interdisciplinary care team, as needed, to implement and monitor the plan of care. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and their families. Acts as a preceptor/resource person and assists others in implementing nursing care in a manner that contributes to the excellence of the nursing department and maximizes teamwork.Advocates for resident's rights. Identifies and refers resident needs to Ethics Committee as needed.Promptly responds to or escalates complaints or investigations.  Demonstrates creativity in solving problems and looking at situations in a new and innovative ways.Interacts with residents, families, co-workers, managers and other departmental staff in a kind, knowledgeable and friendly manner.Collaborates with Nurse Managers to accomplish resident and facility goals.Participates in orientation of new staff and performance evaluations of nursing staff.Follows proper infection control procedures at all times.Remains calm in emergency or stressful situations.Complies with all regulatory requirements and MVHS policies. Actively participates in performance improvement and continuous quality improvement (CQI) activities. Performs related duties as assigned. Education/Experience RequirementsREQUIRED:Two years' experience in a hospital, rehabilitation or skilled nursing home.  Ability to demonstrate competence in technical and manual skills, according to the scope of practice for RNs.Good clinical assessment skills. Ability to lead, guide and motivate others.Good interpersonal skills.Basic computer knowledge.   Licensure/Certification RequirementsREQUIRED:Registered Nurse with licensure in NYS.BLS certification.DisclaimerQualified applicants will receive consideration for employment without regard to their age, race, religion, national origin, ethnicity, age, gender (including pregnancy, childbirth, et al), sexual orientation, gender identity or expression, protected veteran status, or disability.Successful candidates might be required to undergo a background verification with an external vendor. Job DetailsReq Id  90952 Department  CLARK NORTH Shift VariableShift Hours Worked  7.50FTE 0.94 Work Schedule  HRLY NON-UNION-RNEmployee Status A1 - Full-Time Union Non-UnionPay Range 26.68-49.74 hourly
