Summary
JOB SUMMARY
Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of Carroll Hospital and Carroll Hospice. Under the supervision of the Hospice Manager and in cooperation with Hospice Executive Director and the Hospice Medical Director facilitates the admission of patients to hospice and conducts hospice consults to evaluate for hospice program eligiblity. The Admission RN is also responsible for obtaining signed conent forms and conducting an initial nursing assessment of a potential hospice patient and reporting the patient's condition to the Hospice Medical Director using the applicable Local Coverage Determinations. The Admission RN wil collaborate with the interdisciplinary team to determine the patient plan of care. The Admission RN will document the intial assessment, orders, and initial plan of care in the patient's chart utilizing the electronic medical record at the point of care. The Admission RN will act as an advocate for Hospice patients admitted to the Hospital, ensuring the Hospice plan of care is maintained during the in-patient admission and participate in discharge planning.
REQUIREMENTS
Required Associate's Degree Successful completion and graduation from an accredited school of nursing.
Preferred Bachelor's Degree
2-4 years Requires minimum of two years of recent medical/surgical or equivalent nursing experience, one year home health or hospice experience preferred, expertise in performing health assessments.. Required
CHPN - Certified Hospice Palliative Nurse
CPR - AHA Healthcare Provider

SPECIFIC REQUIREMENTS
Interpersonal skills (oral & written); to educate and comfort patients & families; maintain effective contacts with a variety of hospital personnel. Ability to maintain appropriate documentation. Fleet Safety Requirements. CHPN certification should be within 4 years of hire or after meeting CHPN exam requirements of 500 hours of hospice and palliative nursing practice in the most recent 12 months or 1000 hours of hospice and palliative nursing practice in the most recent 24 months (required to apply for CHPN exam).
