Summary of Job Purpose
The Registered Nurse, in collaboration with the Team, is responsible for providing quality patient care by demonstrating clinical competence, participating in ongoing quality assessment, and providing direction to ancillary staff in an acute care facility. Participates in transformational partnership integrated health care delivery model.
Essential Job Responsibilities
Patient Assessment - Performs a thorough and ongoing nursing assessment, including appropriate admission assessment of patient and support systems. Identifies and prioritizes current and potential problems, developing a care history and evaluation and aid in development of an individualized treatment plan. Documents identification of problems on admission on the Health Screening. Re-evaluates problems as needed. Develops appropriate intervention and desired outcomes for all identified problems.
Patient Care/Discharge Planning - Collaborates as part of a multidisciplinary team member with patient and family/significant others and agrees to meet patient's present and discharge needs. Evaluates, revises, and documents using the treatment plan and subsequently reviews the patient.
Teaching - Works independently in the assessment of psychotropic medication regimens. Demonstrates a thorough knowledge of psychotropic medication. Able to teach staff and patients the essentials of psychotropic medications.
Communication - Demonstrates the ability to process and interface appropriate information between the health care team and the patient, family/significant other to ensure quality patient outcomes. Works with insurance companies and other health care providers in the acquisition of authorization and in administering a discharge plan.
Treatment/Procedures - Performs all treatments and procedures in accordance with established Policies, Procedures, and Patient Care Standards. Provides case management as necessary in the coordination of financial, residential, mental health and vocational services for the recipient
Documentation - Accurately documents pertinent patient information, utilizing the nursing process on all required forms, to ensure optimal patient outcomes.
Medication Administration - Provide to the patient the following services: administration, monitoring, pre and post assessment, teaching, evaluation of the patient's response to the medication administered via any route.
CQI - Participates in data collection assurance, i.e., monitoring, studies, research. Understands process and impact of CQI program.
Unit Specific Competencies
All new hires will agree to cross train and float to other Mental Health units as needed.
Review and sign off on all LMP orders daily.
Performs eligibility checks on insurance payors for referrals and admissions.
In conjunction with Patient Finance Department, collects insurance data and ensures accuracy and completeness.
Documents in hospital care management program financials, payor requirements, clinical documentation and authorization status.
Perform concurrent reviews and discharge reviews with payors to ensure authorizations for continued inpatient stay or transfer to the next level of care.
Monitor activities related to Length of Stay issues and concerns.
Serves as liaison between the physicians, treatment teams, patients and 3rd party payers when applicable.
Performs chart audits to ensure compliance with OMH and DNV regulations.
Maintains environmental safety including but not limited to fire safety, reducing fall risks, and removal of expired food.
All new hires will agree to provide on call services on a rotating basis following a three month orientation period.
Other duties as assigned by Supervisor.
Essential Job Qualifications
Education:
Graduate of an accredited school of nursing, currently licensed with R.N. in the State of New York, with 3-5 years of psychiatric experience preferred.
Experience:
Basic knowledge of nursing theory and practice. Knowledge of appropriate technology and practice standards for area of responsibility. Demonstrates sound clinical skills; knowledge of Hospital structure, objectives and standards of practice, positive communication skills. Knowledge of Partial Hospitalization goals.
Training/License:
New York State Registered Nurse license required
