Rn, Case Management Coordinator

Harrison County Hospital is seeking to hire a Registered Nurse Care Coordinator.
This position as a registered nurse or licensed practical nurse with experience in the hospital setting is responsible for ensuring that all assigned patients meet admission requirements for acute care; have appropriate DRG/length of stay identified and continued stay reviews. The job also includes Discharge Planning, Social Services, Case Management and other activities to ensure that specific patient outcomes are achieved.
Position Available: Full-time, Days, 40 hrs/wk (4 days/wk)
EDUCATION/EXPERIENCE: RN Graduate of School of Nursing Case Management experience required. 3 yrs physician office, emergency room or hospital nursing area experience. Knowledge of Healthcare financial systems SKILLS: Ability to abstract medical chart contents Computer literacy for P.C. and hospital information systems Knowledge of current medical terminology and procedures PRO and insurance company standards and criteria Ability to establish smooth working relationship with physicians, their office staff, and ancillary hospital staff.
