As a Case Manager, you'll be a key player on our care team, guiding residents from admission to discharge with personalized care and support.
You'll take the lead in assessments, care planning, and education, ensuring our residents feel confident and well-prepared for their next chapter.

What You'll Do: Welcoming Presence: Be the warm and friendly face that greets our residents, families, and visitors, creating a positive and supportive environment.
Resident Advocate: Conduct meaningful interviews and craft personalized discharge plans within 48 hours of admission.
Care Coordinator: Be the vital connection between all care team members, ensuring seamless communication and collaboration.
Education Champion: Empower residents with the knowledge they need to manage their health, medications, and medical equipment, using the Teach Back Method to boost their confidence.
Advance Directives Guide: Assist residents and families in understanding and obtaining Advance Directives and Power of Attorney documentation.
Insurance Liaison: Partner with our Therapy team to provide timely insurance updates and expertly handle any appeals.
Discharge Planner: Prepare detailed discharge plans, coordinate post-discharge care, and arrange follow-up appointments and services.
Team Leader: Lead and support our unit staff, fostering a collaborative and caring team environment while ensuring the highest quality of care.
Daily Living Assistance: Offer compassionate help with daily activities, ensuring that our residents feel dignified and comfortable.
Safe & Caring Environment: Maintain a safe, clean, and nurturing atmosphere for residents and staff, adhering to all safety and sanitation procedures.
Lifelong Learner: Participate in educational workshops and in-service programs to keep your skills sharp and stay ahead in your field.

What We're Looking For: A graduate of an accredited School of Nursing with a current RN license in Ohio.
CPR certification and a minimum of 3 years of experience in healthcare, long-term care, or skilled nursing.
Strong communication, organizational, and interpersonal skills, with the ability to work independently and as part of a team.
A compassionate, empathetic, and patient-centered approach, especially in challenging situations.
Physical ability to meet the demands of the job, including lifting and transferring residents.
Your Work Environment: You'll report to the Director of Nursing, working in an environment that brings new challenges and rewards every day.
You may interact with emotionally upset residents and families, navigating these moments with grace and empathy.
While exposure to infectious conditions is possible, your safety and the safety of our residents is always our top priority.
Flexibility is key-you'll sometimes work beyond normal hours, including weekends and holidays, to ensure our residents receive the care they deserve.
