Lyra Caregiver Assistance program - Up to 25 counseling or coaching sessions per eligible member per year Accountable for direct and indirect quality care for resident, according to the individualized needs of the resident, following the nursing process and in accordance with the Nurse Practice Act.

Coordinates resident care activities with other members of the health care team, and supervises the nursing care given.

Required Qualifications: Graduation from an accredited school of nursing.
Oregon Registered Nurse License upon hire.
National Provider BLS - American Heart Association within 30 days of hire.
In Compliance with the Department of Human Services (DHS), a criminal background check is required for this position upon hire.
Preferred Qualifications: 1 year of experience in long term care.
