Manage complex clinical situations independently. Collaborate with both licensed and non-licensed team members. Monitor and manage acute and chronic conditions in coordination with Primary Care Providers (PCPs). Administer treatments, procedures, and medications as prescribed. Conduct regular home visits for care, including wound care, lab draws, and other nursing tasks. Qualifications Bachelor s degree in Nursing preferred; Associate s degree required. RN Nursing Diploma or Degree from an accredited School of Nursing or University. At least one year of experience working with the frail, elderly, or long-term care population. Experience in home care and wound care preferred. Current CPR certification by AHA or willingness to obtain. Specific Requirements Reliable transportation, a valid Michigan driver s license, and car insurance. Age 18 or older.
