Job Summary

The Registered Nurse plans, organizes, and directs hospice care, emphasizing community health education and experience.
As a professional nurse, you will utilize all elements of the nursing process to provide comprehensive and compassionate care to patients and their families within their homes and communities.


Duties & Responsibilities Patient Care Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
Provide professional nursing care using the nursing process.
Develop care plans that establish goals based on nursing diagnosis and incorporate palliative nursing actions.
Administer medications and treatments as prescribed by the physician.
Assist patients with activities of daily living and facilitate their efforts towards self-sufficiency and comfort care.
Act as a Case Manager and coordinate patient care for assigned caseload.
Communication Maintain accurate clinical notes regarding patient's condition and care given.
Communicate with physicians and community health-related persons to coordinate the care plan.
Teach patients and families self-care techniques and provide medication and diet instructions.
Provide a safe environment for patients and ensure continuity of care.
Work with the interdisciplinary group to meet the emotional needs of patients and families.
Attend interdisciplinary group meetings.
Additional Duties Participate in on-call duties as defined by the on-call policy.
Supervise ancillary personnel and delegate responsibilities when required.
Maintain and upgrade professional knowledge and practice skills through continuing education.
Actively participate in quality assessment performance improvement teams and activities.
