Schedule Details: M-F 0800 to 1630. No weekends. Two holidays per year- one major, one minor. Back up on call rotation approx. 4 times per month with occasional on call to cover night RN time off.

Our highly skilled team provides a wide array of services, including medication education and management, injections and intravenous therapy, wound care, nutritional counseling, bowel and bladder management and much more. Physical, occupational, and speech therapists address home safety concerns, strengthening and rehabilitation needs. Our licensed social workers offer assistance with community resources, financial assistance, and supportive care and counseling to patients and families.
What You Will Do:The RN Case Manager works collaboratively with patients, social workers, physicians, other practitioners, caregivers, and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment. Assesses, plans, facilitates, and evaluates timely coordination of high quality, cost-effective care. Responsible for improving the health of patients dwelling in their own homes. This requires a nurse who is comfortable working independently, enjoys teaching, is a critical thinker, and has strong communication skills. The RN Case Manager provides hands-on care in the home environment and offers autonomy, flexibility, and the luxury of caring for one patient at a time.
Ideal candidates also possess negotiation skills along with strong analytical and computer skills. Experience with pre-acute and post-acute care as well as community resources is preferred. Case Managers also promote safety, cost efficiency, and a commitment to quality improvement.
What you will need:Associate's Degree in Nursing required. Bachelor's Degree in Nursing preferred.Current State of WI or eNLC compact state RN license.Valid State of WI driver's license with an acceptable driving record.Current BLS (CPR) certification or ability to obtain upon hire.Clinical experience preferred.EPIC software experience preferred.About Us:Learn more at ProHealthCare.org/Careers.
