This is a full-time position, scheduled Monday through Friday, 7:30am to 4:30pm (Phone Triage hours), No nights, weekends, or holidays. This RN will be responsible for a high volume of patient phone call messages that come into our clinic message pools for triage services to be evaluated for either an in-office visit with the provider or at home treatment using the Triage resources available to assist in the best at home care. Documenting in the patient's EMR all that was advised. Incoming messages are not limited to patient care but do include other request for instance paperwork to be completed or specific medical record information like immunization history, mediation refills, referrals to specialist, etc. This position supports other department pediatric clinics with their message pools when needed. Other duties associated in this role is working with the Right Fax looking for documentation patients or outside entities have faxed in for completion. Provider contact: working closing with Providers in consulting them for treatment of their patient's medical needs, collaborating with clinical peers, RNs, LPNs, and MAs to complete tasks. Minimal duties in the clinic setting which would include Rooming patients, EMR charting, assisting LPNS, RNS and Providers with procedures-workflows. POSITION SUMMARY This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients. CORE FUNCTIONS 1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members.
