Visit nchjobs.org to learn more.JOB SUMMARYThe RN Navigator Pulmonology, Critical Care, and Sleep works to independently plan, implement, and evaluate nursing care in patient care ambulatory setting using standards of care, policy, and procedure in conjunction with the providers' oversight and exercises critical judgement. Provides direct nursing care to patients, assumes teaching and leadership responsibilities for PLN Clinic and the ILD nurses, as well as completes the CPAP Mask fittings and teaching for patients. Participates in shared decision making, referrals and Pulmonary, CC, and Sleep activities. ESSENTIAL DUTIES AND RESPONSIBILITIESOther duties may be assigned. Coordinates the referral process and introduction of patients into the PNC, ILD, and Sleep Programs with the potential to coordinate care in future clinical trials. · Employs patient teaching to promote health and safety. · Evaluates progress toward outcomes. · Practices ethically. · Receives incoming referrals from physicians and hospitals and responds quickly in order to ensure a rapid access into the system. · Triages referrals according to written criteria to assure those patients' needs are met on a priority level. · Communicates back to referring physicians and hospitals the plans for acceptance into the program in an effective manner · Educates the patient and family regarding the referral process, steps of evaluation, and keeps patient and family informed throughout the entire process. · Demonstrates leadership in the professional practice setting. · Collaborate with patient, family, the inter-professional team, and others in the conduct of nursing practice. · Utilizes appropriate resources to plan and provide nursing services that are safe, effective, and financially responsible. · Practices in an environmentally safe and healthy manner. Works in collaboration with the providers and clinic staff to assure a smooth transition of patients during preadmission, hospitalization, preparation for discharge and outpatient services. · Oversees Pulmonary Lung Nodule Clinic (PNC) screenings to coordinate for free screening CTs to ensure that program is meeting stated guidelines. · Oversee both PNC and Interstitial Lung Disease (ILD) nurses to provide support. · Acts as a liaison to the PNC committee, sleep center and associates involved in sponsored clinical trials (i.e., data submission, monthly webinar participation, etc.) · Follows all protocols for documentation and maintenance of databases related to the Pulmonary Lunch Nodule Clinic (PNC). · Facilitates testing for oxygen including ordering and or preforming 6 min walk tests. · Assists and delegates to internal staff all general duties, including refills, rooming patients, 6-minute walks. · Facilitates patient instruction on medication including delivery devices such as nebulizers and inhalers. · Disclose results of sleep testing to patients and coordinate further care including second studies or ordering equipment. · Assess patient's CPAP Mask fit and make changes as needed. · Facilitate in person CPAP teaching for patients. · Facilitates all Durable medical equipment (DME) orders leaving the office for Oxygen, sleep equipment, Nebulizers etcetera. · Maintain a list of insurance companies accepted by each DME Provider. · Facilitates all Medical equipment annual detailed orders that must be hand signed by providers and returned. · Facilitate changes in home durable medical equipment settings. · Liaison between medical equipment companies and physicians to ensure that patients get timely and appropriate equipment. · Develops and implements educational materials for patients and families to ensure consistency in information presented, taking into consideration their learning needs and barriers to learning, and documents the education provided. · Responsible for overseeing internal staff education and competencies and for serving as a resource to all nursing units for the care of this patient population. · Responsible for overseeing internal staff competencies regarding sleep compliance downloads. · Maintains currency with all required compliance training programs. · Assists in the coordination of a post-discharge clinic to evaluate patient progress towards expected outcomes and to identify any care issues. · Coordinates specialty medications initiation paperwork. · Delegates to ensure that daily monitoring of medication refrigeration, emergency box, sample closet, and logbooks are all up to date. · Manages all incoming and outgoing surgical clearance requests in a timely manner. · Identifies potential topics for research related to the program. · Keeps up to date on all current literature and studies which pertain to PLN Program and serves as the primary resource to share this information with others. · Provides written reports as requested to support research projects. · Maintains a database of clinical protocols for reference and review on a recurring basis (at least annually) to assure quality outcomes. · Follows all protocols for documentation and maintenance of databases related to the PLN Program. · Participate in and coordinate outreach to current and potential referring physicians. · Maintains currency with all required compliance training programs. · Maintains a commitment to professional growth and competency including pursuit of related certifications. EDUCATION, EXPERIENCE AND QUALIFICATIONS· Minimum of 2 years clinical RN experience with patient care required; Pulmonary, Sleep or Critical Care experience preferred. · Licensed as a Registered Nurse (RN) in the State of Florida. · Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association. · Communication and Problem-Solving skills and ability to maintain flexibility. · Ability to manage, read, and direct patient care and administrative functions in pulmonary and crucial care services. · Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
