This RN works directly with Medicaid & Medicare patients in the community on the MSHO program, assessing them in their homes and helping connect them with medical care and services. Will chart and document from home when not performing face to face assessments. Assessments are questionnaire and information based rather than physical assessments. The ideal candidate for this position works independently, is comfortable in others' homes, is organized, manages time well, and is willing to ask questions when faced with unfamiliar situations. This position is a hybrid remote role that requires face to face assessments, so the candidate must reside in the service area. Essentia provides needed technology, and the candidate is responsible for reliable internet service and an office space, as well as reliable transportation. Mileage is reimbursed at the current IRS rate, and the position is salaried. This employee serves as a float position throughout the whole state of Minnesota. This nurse primarily works from home, but travel- including occasional overnight travel- will be required.Education Qualifications:

What You'll Do:
Minnesota Senior Health Options (MSHO) Care Coordinator duties include responsibility for assisting members enrolled in a dual eligible Minnesota Health Care Programs to make informed choices about their health care services, conducting assessments, service coordination, and assisting members in making progress towards his or her health goals and coordinating care with the interdisciplinary team. This includes making in-person visit to enrollees' home.
Licensure/Certification Qualifications:
Current Minnesota RN license required upon hire.Must have reliable transportation and a valid driver's license.Experience in community nursing, such as home care, assisted living, case management, outpatient care, etc. is required. Should not be a new grad and must have experience beyond the hospital setting.

Compensation Range: $69077 - $103626 / year
