Connect with your Talent Advisor for additional specifics.Responsibilities Serve as a patient navigator throughout the continuum of care.Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.Coordinate efforts for treatment and promote quality improvement and educational efforts.Compile data, track outcomes, and make recommendations for process improvement.Provide referrals/support for follow-up care for patients who are uninsured or underinsured.Requirements Licensure / Certification / Registration:Licensed Registered Nurse obtained prior to hire date or job transfer date required.Registered Nurse credentialed from the Oklahoma Board of Nursing required.BLS Provider preferred. American Heart Association or American Red Cross accepted.Education:Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.Additional Preferences Must be willing to obtain a compact license within 90 days of hirePrior Care Manager, Case Manager in Home Health/Hospice, or Patient Navigator in Ambulatory settingWilling to travel within 40 mile radius of office for in-field component conducting patient home visits and attending specialty appointments (30%)Why Join Our Team Ascension St. John has been serving Northeast Oklahoma with compassionate, personalized care since 1926.
