CareChoice Cooperative in MN.Job DescriptionThe Director of Clinical Development will work collaboratively as a part of a leadership team to develop and implement key elements of a standardized Model of Care (MoC) in participating skilled nursing facilities. This MoC will be supported by the development of a remote clinical coordination system designed to facilitate timely clinical intervention and support for patients at risk of hospitalization. The primary role of the Director of Clinical Development will be to identify and implement innovative strategies and clinical solutions to support project efforts. This will include developing standardized care paths, transition processes, communication protocols, support systems function and management of the clinical coordination team.An advanced clinical skill set specific to the nursing home environment is essential. This is a 1.0 FTE position with competitive salary and benefits. This position requires a combination of work from an office in Roseville, MN and regular travel to participating locations within the Minneapolis/St. Paul, MN metro area.
