Obesity impacts over 40% of the U.S. adult population today and is projected to affect more than 50% by 2030. Historically, only about 1% of patients have received appropriate medical treatment for this condition, but the field of obesity medicine is now entering a period of rapid growth. Form Health is on a mission to provide compassionate, accessible, evidence-based obesity and weight management clinical care. Our virtual clinic ensures that science-based treatments—including GLP-1 medications—are accessible and used effectively to deliver sustainable outcomes at manageable costs for patients, employers, and healthcare plans.
Form Health patients work one-on-one with an obesity medicine doctor and registered dietitian to achieve their health goals. The Form Care Team creates a personalized plan that addresses lifestyle changes and, where appropriate, includes a prescription for FDA-approved medications. All care is delivered virtually, making it accessible to everyone. Patients use the Form app to connect with their Care Team through video visits, send messages, track their progress, and access educational resources. We are covered by most major insurance plans, including Medicare.
We are looking for a passionate and empathetic AI/LLM Senior Product Manager with hands-on experience to lead the roadmap and build iterative AI-driven experiences for our internal users (practice managers, dietitians, providers, care coordinators), and eventually our patients. This role is critical to scaling the number of patients we serve 10x year-over-year, enhancing both the patient and operational experience. You will partner closely with design, engineering, and cross-functional stakeholders to assess user needs, prioritize them, and work iteratively to deliver impactful solutions.
What you will doDeeply understand both internal and patient-facing processes at Form Health and identify impactful opportunities for AI/LLM use, prioritizing key needs through research and user feedback.Run experiments to validate promising use cases and bring successful experiences to production.Partner with engineering to define and set up the necessary infrastructure, delivering prioritized work in a timely manner, and iterating on high-value use cases.Vet and integrate the most appropriate vendors and solutions to enhance our AI capabilities.Define the AI product experience, vision, strategy, and roadmap in alignment with company goals. Set and measure KPIs to ensure success and iterate to drive continuous improvement.Collaborate with cross-functional leaders to define, prioritize, and deliver solutions iteratively, influencing broader roadmaps and ensuring alignment across teams.Advocate for the product within the organization: lead product demos, training sessions, and presentations, and serve as a liaison among stakeholders.About youHands-on experience prototyping and delivering level LLM based experiences5+ years of experience building enterprise products, focusing on complex workflows and iterating based on measurable progressStrong technical skills to understand APIs and workflows across both in-house and third-party softwareExperience evaluating third-party vendors and integrating solutions across multiple systemsA track record of defining product vision, rallying support around strategic goals, and delivering iterativelySelf-driven, adaptable, and excited to learn new skills in a rapidly growing startup environmentPassionate about making compassionate, evidence-based healthcare accessibleStrongly preferred:Understanding of healthcare, and specifically experience with care deliveryBoston based OR open to maximize overlap with EST hoursAbout Form Health cultureWe are deeply invested in our core value to put patients first, and also deeply committed to creating a culture where every employee is valued and we learn and improve together.
