Support andmonitorShared Living Providers to ensure necessary resources for individuals living in their homes.
Manage caseload of individuals living in Shared Living.

This position will serve Malden, Lowell, and Boston.
Responsibilities
Demonstrate knowledge of, and commitment to, agency mission and values.
Coordinateall services to ensure thateach individual'sneeds are being met.
Schedule and attendmonthlymeetings with Shared Living Providers and individuals receiving support.
Communicate effectively with DDS Service Coordinators, families,guardiansand all other parties involved in supporting the individual.
Support the SLP toassistpeople in living full lives; including social, spiritual, cultural,recreationaland personal endeavors.
Ensure that the SLPpromoteindividual'shealth and well-being by insuring access to health care andopportunitiesfor a healthy lifestyle and choices.
Monitor and oversee the implementation andtimelydocumentation of each person's Individual Service Plan.
Complete all required documentation foreach individual'sISP in HSCISin a timely manner.
Ensure that each individual lives in a home that is clean and safe.
Monitor the SLPuse of respitein accordance withthe budget.
Monitor the required certificationsfor SLP and their RespiteProviders, andprovide assistance to schedule required trainings.
Monitoreach individual'sfundsin accordance withtheir funds management plan and HMEA's financial procedures.
Participate in Shared Living team meetings and Provider Forums.
Assist Shared Living Director to ensure regulatory compliance.
Maintain required agency documentation.
In conjunction with Shared Living Director, develop departmentplanand strategies and evaluate results.
Participate in agency-wide projects.
Provideon-call support to SLP when scheduled.
Maintain current confidential file for individuals and update CID accordingly.
Performs otherreasonably relatedduties as assigned.

Qualifications
Bachelor's degree in related field preferred; significantadditionalexperience may be considered in lieu of degree with approval.
Threeyears' experiencein DS/MH area.
Experience in training and system monitoring.
Valid driver's license along with an acceptabledriving status and reliable vehicle.
Additionalspecific experience may berequired.
