We're also certified by the Joint Commission as a Primary Care Medical Home and a Behavioral Health Home.Position SummaryThe Shift Monitor will support the daily activities of a recuperative care facility service unhoused patients who are recovering from illness.
The Shift Monitor assists with patient needs, knowing their locations and activities within the facility, notes any changes in patient care and reports it to nursing.
The Shift Monitor supports clinical teams with admission and discharges as well as general facility management, including laundry and cleaning.Required Qualifications High School diploma or equivalent, or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
Nursing Assistant Certificate preferred.Minimum of one year related experience.
Experience with the unhouse population desired.Current First Aid and CPR Certification.Ability to maintain detailed records.Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.Good interpersonal skills and ability to remain calm when working with individuals experiencing mental health disorders.Computer (PC) literate in Microsoft Word, Excel, Outlook and Teams.Able to communicate effectively in English, both verbally and in writing.
Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.Assist and support the Center/Department/Program to meet standards of High Reliability.Essential Responsibilities Support clinical teams with admitting patients (bag checks, reading beds, etc.)
and discharging patients.
Launder bedding and towels, etc.
Communicate with team through Outlook and Teams.Assist Licensed Vocational Nurse and Certified Nursing Assistants with related paperwork as needed.Monitor client bed count and facility safety; call 911 as warranted.Clean bathrooms, offices and client spaces.Clean kitchen: wash dishes, pots, pans, utensils, wash down countertops and stove; empty trash; clean client refrigerator weekly, defrost at least monthly.
Support patients and staff with meals and snacks.
