Skilled Nursing Professional

Post Acute Care Opportunities We are currently seeking a skilled and compassionate Licensed Practical Nurse to join our Post Acute Care team.
As a LPN, you will be responsible for providing high-quality nursing care to our patients, working closely with our interdisciplinary team to ensure the best possible outcomes.
Job Summary:
Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
Collaborate with the care team to develop and implement individualized care plans.
Conduct thorough assessments of patients' physical and emotional needs.
Develop and implement strategies to promote patient independence and self-care.
Requirements:
Graduate of an accredited LPN program.
Current LPN license or compact license recognized by the State of Vermont required.
Must possess strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
Preferred Qualifications:
Previous long-term care experience.
Basic computer skills and ability to learn electronic medical records.
