Responsibilities
• Act as a liaison and representative of the residents' social interests• Assist in planning, organizing, and developing the Social Services Department• Ensure the medically-related emotional and social needs of the residents are met• Maintain resident right reviews and advance directives• Participate in community planning and development of social care plans• Provide information to residents and their families about support agencies and financial assistance programs• Assist with the admission process• Maintain a professional and respectful relationship with community contacts• Support the Mission, Values, and Vision of NHS Management, L.L.C. and the facility• Report instances of abuse, neglect, and other concerns
Requirements
• High School graduate or equivalency• Willingness to help others• Positive communication skills• Patience, tact, enthusiasm, and a positive attitude towards the elderly and disabled• Must be at least 18 years of age• Must be able to speak, read, write, and understand English• Capable of performing essential job functions and physical and sensory requirements
