Description The role of the Social Services Clinician is to work with and support patients through the provision of information/education, resources, advocacy, and referrals. This position assists patients and families to navigate systems of care utilizing personal, community, and governmental resources. The comprehensive impact of behavioral and physical health conditions on the client and family will be addressed in partnership with providers to meet their various social service concerns.
