Social Services Designee - Acute Care

Be part of a great Team within a great organization – The Social Services Designee is responsible for providing medical social services to the patient and family in accordance with the physician's plan of care within the guidance of State and Federal Regulations. The role of the SRMC Social Services Designee is to evaluate patients, determine their needs, and formulate an action plan to ensure that those needs are met efficiently. The SRMC Social Services Designee will work with all customers throughout the Pole Creek Campus that require assistance, but the primary role is to collaborate in the discharge plan of the hospitalized patient.
Job Responsibilities: Provides Medical Social Services to the patients/ residents and their families in accordance with the physicians with the guidance of State and Federal Regulations. The role of the SRMC Social Services Designee is to evaluate patients, determine their needs, and formulate an action plan to ensure that those needs are met efficiently. The SRMC Social Services Designee will work with all customers throughout the Pole Creek Campus that require assistance. Provides Resident/Staff care and Social and Spiritual Needs Performs and maintains documentation. Communicates effectively. Promotes and ensures patient safety in performance of all responsibilities. Assists/ implements Admit/Discharge planning services, human resources and financial goals and objectives consistent with Acute Care's goals and objectives. Follows HIPAA regulations and ensures that the confidentiality of patients' medical, personal, financial records, employee records, and organizational records are maintained. Complies with the state and federal regulatory requirements related to the performance of operations and requirements of the company Compliance Program. Participates in quality assurance, performance and quality improvement initiatives, and activities which support the facility and department operations. Education & Experience: Education: Bachelor's degree in social work or social services designee certification. SRMC may support the right candidate with social work experience with obtaining the designee certification within 6 months of hire.Experience: Prefer at least 2 years of experience in the medical social work field (acute care and geriatrics).Required licensure/certification: Must have a current and valid certification as a Social Services Designee in the State of Nebraska.
