Provides social work services to meet the needs of residents and families. Completes accurate documentation in medical records per regulatory requirements.

Responsibilities

Advocates for residents rights, family/guardian, and organization in compliance with State and Federal regulations.

Manages the social work function of resident assessment and care plan process per regulatory requirements.

Participates in initiatives and committees for quality improvement of facility.

Provides social work services to meet needs of the residents and their family/guardian.

Responsible for admissions and discharges process of new residents to facility to include verifying payer sources.

Responsible for marketing of facility as related to management of census.

Qualifications

EDUCATION:

Bachelors, Social Work, Required
EXPERIENCE:

1 year long term care, Required
2-3 years long term care, Preferred
