Social Worker

We are looking for an experienced SW with Skilled Nursing Facility experience to join our dynamic interdisciplinary team at a 124 bed facility in Amesbury, MA.
Experience with MDS, Care Plans, discharge planning, leveling is required.
Looking for a motivated candidate that can take charge and demonstrate creative, customer service oriented skills functioning independently, as well as part of a team, to achieve positive




Responsibilities


- Conduct patient assessments and develop individualized care plans.


- Provide behavioral therapy and counseling to patients.


- Maintain accurate medical documentation and records.


- Collaborate with interdisciplinary teams for discharge planning.


- Utilize premium skills such as psychotherapy and motivational interviewing.


- Ensure patient confidentiality and compliance with HIPAA regulations.




Job Type: Part-time




Pay: $37.00 - $42.00 per hour




Expected hours: 16 – 24 per week








Experience:


* Social Work: 1 year (Preferred)




License/Certification:
