Westminster-Canterbury of the Blue Ridge (WCBR) is seeking a team member to join our Resident Services department supporting a lively 57-acre campus, 450+ residents, and 300+ associates. The Independent Living Social Worker at Westminster-Canterbury of the Blue Ridge (WCBR) provides support and resources needed to enhance the well-being of the residents, families, and those involved in their lives.
\n ResponsibilitiesAssist and support the psychosocial needs of Independent Living residents, primarily during critical transitions involving losses associated with cognitive and/or physical decline while residing in Independent Living,Assess and respond to psychosocial needs of Independent Living residents and their families; collaborate with the social worker in HC and the social work coordinator in CL during transitions as well as dining, activities, etc. to meet identified needsProvide short-term individual and family counseling, and group work as appropriateServe on committees  and support groups; arrange for educational opportunities to address resident concerns; Attend IDT meetings and care plan meetings for residents temporarily in Healthcare, Assisted Living, and VistaProvide community referrals to community resources as needed QualificationsThe ability to establish rapport quickly and communicate effectively with residents and their familiesThe ability to function effectively as a member of the Interdisciplinary TeamThe ability to conduct brief, evidence-based screening and to conduct or arrange for more detailed assessments when indicatedThe ability to create and implement integrated care plansPossess strong interpersonal skills including relationship building with associates, residents, families, and the greater Charlottesville community RequirementsCurrent LCSW in the state of Virginia with Master's Degree in Social WorkRelevant experience with the aging population and their familiesExperience in a healthcare settingKnowledge of community resources Benefits SnapshotPaid Time Off packageEducation Program to offer financial assistance with education costsRetirement Savings Plan with a company matchFree Meal while workingPaid HolidaysFree gym and pool accessFree parkingMedical, Dental, and Vision Benefits
