Social Worker Msw- Case Management

Prepares the patient to transition from the hospital to the next appropriate care setting based on their individual needs.
Use their understanding of medical conditions, insurance coverage and how physical/psychosocial injury may affect the patient once a patient leaves the medical setting to plan and initiate an individualized discharge plan.
Provide clinical information to payers and/or post-acute providers in order to obtain authorization for hospitalized patients.
This is an exempt position.


Qualifications

Education: Master's Degree in Social Work

Experience: Three years' experience in a clinical setting.


Licenses/Certifications: Masters in Clinical Social Work required.
Licensed Clinical Social Worker desirable.
BLS required.
