As a Specialty Nurse Navigator, you will be responsible for: vFormulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. vEstablishes specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance. vAssists patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Identifies and aligns resources that impact high-risk care. vCoordinates services on behalf of the high-risk patients. vWork in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed.
