As a Specialty Nurse Navigator, you will be responsible for: Navigating patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient.Establishing specific, measurable, attainable, relevant, and time-specific nursing goals to guide the care planning process and promote self-reliance.Providing in-person clinic visits as needed at the initiation of care and throughout transitions of care across all of our hematology/oncology clinic sites.Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.Providing disease and treatment-specific education tailored to meet patients' individual goals of care.Onsite rounding throughout treatment plan implementation as needed to provide necessary in-person patient evaluation.Providing direct, hands-on nursing care, as needed.Assisting patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance and addressing social services needs by identifying community resources and referring to these programs.Collaboration with the system-wide authorizations team and scheduling in other departments, as needed, to ensure patients receive timely and comprehensive care.
