Establish specific, measurable, attainable, relevant, and time-specific nursing goals to guide the care planning process and promote self-reliance.
Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance.
Identify and align resources that impact high-risk care.
Coordinate services on behalf of high-risk patients.
Work in collaboration with the healthcare team to track and review data for research projects as well as resource utilization related to relevant specialties and make recommendations as needed.
