Description Responsibilities Demonstrates commitment to the Mission Statement of the hospital and department.
Fulfills record keeping requirements of the department.
Demonstrates good body mechanics at all times.
Schedules patients, meetings, etc., organizing times to meet departmental needs and adjusts the schedule as necessary to accommodate unexpected situations and/or assist other staff with patient care.
Evaluates and screens speech, sound production, resonance, voice, fluency, language, cognition, feeding, and swallowing problems, identifying treatment needs.
Is able to evaluate the developmental needs of patients from infant to geriatric and plan/adjust care, services, and instruction appropriately within the scope of job responsibilities.
Develops and modifies Plans of Care, which include speech diagnosis, assessment of rehab potential, treatment plan, and long/short-term goals, involving patient/family in goal setting.
Provides treatments of speech therapy, swallow treatment, cognitive training, speech generating device training, and non-speech generating augmentation training.
Provides patient/resident/family education and counseling regarding the nature of dysfunction, rationale and proposed treatment, and responsibilities of patient/family, instructing them in self-management programs including care and use of adaptive equipment.
Provides educational in-services and screenings in-house and to community groups.
Accurately documents for each patient according to government regulatory and department standards, ensuring clinical records are descriptive and written with objective and functional goals, describing responses to therapy and progress towards goals.
Communicates with employers, rehabilitation consultants, and insurance companies regarding patient's functional status and rehab potential.
Participates in Program Development, contributing ideas, assisting in protocols, and program development.
Participates in staff meetings, continuing education programs, continuous quality improvement programs, and quality assurance, attending mandatory in-services yearly (CPR, Fire, Safety, Disaster, and Infection Control), and complies with universal blood and body fluid precautions.
Adheres to personnel and departmental policies and procedures.
Exhibits and promotes a positive attitude and outlook in the workplace and the community, demonstrating behavior conducive to the professional operation of the organization.
Participates in and advocates for departmental interdepartmental communication within the facility to facilitate improved customer service and patient outcomes.
Reports to work when scheduled and demonstrates flexibility of personal time when requested to come in on a scheduled day off.
Adheres to facility and departmental safety policies, notifying the supervisor of any safety concerns/hazards to staff and/or patients/residents.
Successfully completes required annual job-specific competency testing.
Uses work time productively, taking care of equipment and supplies properly (cost-conscious use of supplies, disposable/non-disposable/recycled), ensuring breaks and lunches are within the time allotted.
Performs other duties as assigned by the supervisor.
Qualifications: Graduation from an accredited School of Speech-Language Pathology with a Bachelor's Degree and Master's Degree.
Must have a certificate of clinical competence (CCC) through the American Speech and Hearing Association, or be working toward it under a post-graduate clinical fellowship under a certified Speech Language Pathologist.
Must hold an active license from the State of Wisconsin or a permit from the examining board while awaiting state certification.
Must maintain state licensure and American Speech and Hearing certification.
Physical ability to push, pull, reach, squat and lift up to moderately heavy loads.
Ability to grip, squeeze or pinch without restrictions.
Ability to maintain good interpersonal relations.
